چکیده

Macrostructure of sleep During sleep, 5 to 6 sleepcycles occur during the night, lasting approximately 1,5 hours. Every sleep cycle consists of one REM stage, characterised by rapid eye movements, paradoxical cortical activation and muscular atonia, and four non-REM stages (‘macrostructure’ of sleep). The first three to five hours of the night are considered to be most important and responsible for the restorative function of sleep, since during these hours the largest amount of slow wave sleep (non-REM 3&4) occurs. The sleep in the second part of the night is dominated by superficial REM sleep and sleep stage 2.1

Melatonin agonists in primary insomnia and depression-associated insomnia: are they superior to sedative-hypnotics?

Current pharmacological treatment of insomnia involves the use of sedative-hypnotic benzodiazepine and non-benzodiazepine drugs. Although benzodiazepines improve sleep, their multiple adverse effects hamper their application. Adverse effects include impairment of memory and cognitive functions, next-day hangover and dependence. Non-benzodiazepines are effective for initiating sleep but are not ...

Discontinuation of benzodiazepines among older insomniac adults treated with cognitive-behavioural therapy combined with gradual tapering: a randomized trial.

BACKGROUND Long-term use of hypnotics is not recommended because of risks of dependency and adverse effects on health. The usual clinical management of benzodiazepine dependency is gradual tapering, but when used alone this method is not highly effective in achieving long-term discontinuation. We compared the efficacy of tapering plus cognitive-behavioural therapy for insomnia with tapering alo...

Residual Effects of Short Half-life Non-benzodiazepine Hypnotics

Surveys showed that around 1996 the most frequently prescribed hypnotic in the US was zolpidem, while zolpidem and zopiclone were among the five most frequently used hypnotics in Europe [1, 2]. The...

What happens when doctors stop prescribing temazepam? Use of alternative therapies.

We investigated the withdrawal of temazepam in a single general practice using two alternative prescribing policies: an alternative benzodiazepine; or an alternative group of drugs recommended for short-term management of insomnia, including sedative antihistamines and chloral hydrate. The study showed that temazepam prescribing in general practice can be reduced or stopped by using a simple in...
